Previous Dental Records and X-Rays
If you are transferring from another dental office, you may ask your previous dentist to forward relevant dental records and recent X-rays to 525 Dental.
Please contact our office if you need assistance arranging the transfer of your records.
You do not necessarily need new X-rays at every dental visit. Your dentist will determine whether X-rays are appropriate based on your individual dental history and current oral health.
Dental Insurance Information
If you have dental insurance, please bring your insurance information and any forms that need to be completed.

For Parents Bringing Children
We welcome children and families at 525 Dental.
For a child’s first dental visit, we focus on creating a positive and age-appropriate experience while assessing their developing teeth and oral health. The Canadian Dental Association recommends that children have an initial dental assessment by their first birthday or within six months of their first tooth appearing.
Please let our team know if your child has any particular concerns, dental anxiety or special needs before the appointment so we can help make the visit as comfortable as possible.
For patients under 18, please contact our office regarding our current parent or guardian requirements before the appointment.
Will Treatment Be Done at the First Visit?
In many cases, the first appointment is primarily focused on examination, diagnosis and discussing your oral health needs.
